The inpatient residential stay teaches the skills to ensure that you are prepared to leave treatment with a solid foundation to continue the recovery process. Albeit the changes within the body, mind and spirit can be profound, they are also delicate in the early stages of recovery, which is where an aftercare program comes in handy.
Much like when you have a chronic illness, our patients in recovery will have access to a team of doctors and specialists for ongoing maintenance and aftercare of addiction. At Simcoe Addiction and Mental Health, we offer lifelong support.
What is an Aftercare Program?
Aftercare is a type of structured care to help a person stay focused on their recovery goals after that completed a formal addiction treatment program. An aftercare plan is much like a discharge plan from hospitalization to treat a chronic condition like diabetes or heart failure. Although relapse doesn’t happen to everyone it is pretty common. Studies show that rates of return to drug and alcohol use among adults and teenagers who have completed a rehab program can be as high as 80%. This is why aftercare is one of the most important stages of the rehabilitation process.
When treating substance use disorders, aftercare planning must include:
- A thorough understanding of the severity of the condition
- Identification of potential barriers to successful treatment
- Outpatient care expectations
- Appointment setting
- A realistic, sustainable self-management plan
- A protection plan to activate in the event of triggers
- A regrouping plan in the event of recurrence of symptoms
Types of Aftercare Programs
This consists of weekly aftercare meetings that allow you the opportunity to connect with your friends on a weekly basis. This provides an excellent way for you to address things that may not be working or just as a means of connecting with others who are striving for recovery. Weekly meetings will be facilitated by one of our passionate addiction counsellors. These meetings can become a way of remaining accountable in recovery and a place to voice your frustrations or challenges in the earlier stages of recovery.
Back on Track Week
This is a free service we offer to all our residents that may need a refresher. If when you leave, you feel the need to “get back on track” then we will offer you a safe place to do so. This free week of stabilization can be used at any time. Please note that this offer expires after 1 year. During this time, you can reconnect with a therapist and participate in our programming for 7 days. The 7 days will be used to look at the issues that are causing distress in your life. Transitioning services are offered as well. This means that once you’ve completed your residential stay, you can continue to work with a therapist. If you would like to find a therapist closer to your area or maybe you feel additional, more specific care is needed by another professional or organization, we can assist you to get connected with those services.
